Usmle Step 3 Format

The USMLE Step 3 exam assesses your ability to apply medical knowledge in real-life scenarios. It’s the final step toward obtaining your medical license in the U.S.

USMLE Step 3 is a two-day exam. The first day focuses on foundation concepts, while the second day tests clinical management and decision-making skills. 

Exam Duration 

The first day covers multiple-choice questions (MCQs) on general medical knowledge, basic sciences, and patient care foundations. You will encounter seven 60-minute blocks. 

Day 1 - Foundations of Independent Practice (FIP) 

Day 1 of the exam primarily includes topics like internal medicine, psychiatry, pediatrics, and surgery, with a focus on diagnosis and patient management principles. 

Day 1 Content Breakdown 

The second day involves case simulations and additional MCQs. You’ll be tested on decision-making in acute care settings, requiring clinical judgment and management skills. 

Day 2 - Advanced Clinical Medicine (ACM) 

Computer-based case simulations are an essential part of Day 2. These simulate real-life medical scenarios, allowing you to showcase your ability to manage complex patient cases.

Case Simulations (CCS) 

MCQs on Day 2 focus more on patient management. You’ll face both standard questions and extended matching sets, where you must apply clinical knowledge to treatment plans. 

Multiple Choice Questions (MCQs) 

USMLE Step 3 is scored on a 3-digit scale. You must pass both days independently. A passing score demonstrates that you’re ready for unsupervised practice as a physician.
