After completing their MBBS, Indian students cannot practice medicine directly in the USA. To be eligible for residency, which is a board-certified postgraduate program, they must first complete the required medical courses in the USA and pass the USMLE exams with high marks

USMLE preparation for Indian students for Step 1 can begin as early as the second year of MBBS in India, and there are courses provided by institutions such as USMLE Strike, as well as live classes taught by experienced US-based doctors. 

The cost of this route, including 6 months of clinical rotation or clerkship, is around US$30,000, but can be reduced to approximately US$20,000 if the student is able to pass the USMLE-2 CK after only 3 months of clinical rotation.

Option – 2 – MD in USA after MBBS in India (USMLE after PG in India)

Going to the USA and pursuing USMLE after completing PG in India, based solely on a clerkship, has its drawbacks. During the clerkship, doctors can pass the USMLE 2CK exam and become eligible for residency in the USA with a work permit. However, the limited time period for the clerkship may not allow sufficient time for preparation, and failure to obtain residency after passing the exam would require the doctor to return to India and start the process again.

Pursuing a Master’s in Public Health (MPH) in the USA is a more stable and cost-effective option for Indian doctors seeking postgraduate medical education in the USA. An MPH course provides a breathing period for students, as the student visa issued by the US consulate for Indian students is typically for 5 years for a master’s program. Many Indian doctors pursuing USMLE after PG in India can take admitted to an MPH program and use the time period to prepare for their clinical exams. 

This allows them to ultimately earn two degrees, one clinical after completing their residency and one non-clinical, such as a Master’s or Diploma in Public Health. The total cost to complete a medical PG in the USA through this route is approximately US$ 50,000, which includes a 3-month clinical rotation/clerkship. 

There are other advantages of pursuing MPH programs in the USA leading to medical PG such as: Creating a network of doctors and gaining their confidence during this 2 years period. While pursuing an MPH degree in the USA, one can take a break for one semester after the first year and during this period, the doctors complete their clerkship. 

The program allows the student to get 5 years of Visa while going from India and in case of a problem in getting the residency for the clinical PG program, the public health management degree aids in taking care of your living expenses.

Option – 3 – MD in USA after MBBS in India 

Some Indian doctors prefer to skip non-clinical programs like MPH and focus only on their clinical experience. For USMLE preparation for Indian students, this third option is available. 

They can pursue a 2-year USMLE-preferred Green Book clinical rotation (not an observer-ship) at an ACGME-approved hospital, following USMLE-1 coaching and exam. This option costs the same as option-2 but does not include MPH. After completing the rotation, they must secure a residency since their visa would expire.
