The USMLE is the licensing exam for doctors who want to train and work in the United States, while the AMC exams are for doctors who want to work in Australia. In a USMLE vs AMC Exam comparison, the AMC route generally costs less in exam fees and may allow doctors to start paid hospital work sooner, depending on registration and employment requirements. The USMLE route can take longer and is more competitive, but it offers structured specialist training through US residency. The better choice depends on your preferred country, budget, career goals, and how soon you want to start working.

What is the USMLE?
The United States Medical Licensing Examination (USMLE) has three exams:
- Step 1 covers basic sciences like pathology, physiology, and pharmacology. It is scored only as pass or fail.
- Step 2 CK covers clinical knowledge and patient care. It gives a number score, and residency programs look at it closely.
- Step 3 covers managing patients on your own. Most international doctors take it during residency, not before.
Indian doctors can take Step 1 and Step 2 CK at test centres in India. Step 3 is only held in the US.
What is the AMC Exam?
The Australian Medical Council (AMC) assesses doctors who trained outside Australia. Indian doctors usually follow the AMC Standard pathway, which has two main parts:
- AMC MCQ: A computer adaptive test with 150 questions over about three and a half hours. “Computer adaptive” means the questions get harder or easier based on how you answer. It is available all year at Pearson VUE centres around the world, including in India.
- AMC Clinical Exam: A practical exam with 16 patient stations. It is held in person in Melbourne. Some doctors take a workplace based assessment instead, which is done while working in an approved Australian hospital job.
Doctors trained in countries like the UK, Ireland, Canada, the US, and New Zealand may qualify for the Competent Authority pathway and skip the AMC exams. Most Indian graduates do not qualify for this and use the Standard pathway.
Is AMC Easier than USMLE?
There is no simple answer, because the two routes are hard in different ways.
The AMC has fewer exams. You clear one written test and one clinical assessment. The USMLE asks you to pass Step 1 and Step 2 CK before you can apply for residency, and Step 3 later.
The USMLE is more competitive after the exams. Passing is only the start. For Step 2 CK, you need a strong score to compete for residency. In the 2026 Match, only 56.4% of international graduates who were not US citizens matched. For those who needed a visa, the rate was 54.4%.
The AMC Clinical Exam is a real hurdle. It tests how you talk to patients, examine them, and make decisions, in the way Australian doctors practise. Doctors who are strong in theory but less used to this style of assessment often find it challenging.
The AMC MCQ tests general clinical knowledge. The USMLE Step 1 covers a very wide range of basic science in great detail, which many Indian doctors find harder.
In short, the AMC route usually has fewer steps, while the USMLE route has more exams and a tougher competition for residency places.
Which is Cheaper, USMLE or AMC?
USMLE exam fees (2026):
- Step 1: USD 695 plus USD 210 for testing outside the US, a total of USD 905
- Step 2 CK: USD 695 plus USD 235 for testing outside the US, a total of USD 930
- Step 3: USD 955
- In India, 18% GST is added to the international testing fee
You will also pay for ECFMG certification, OET Medicine, residency applications, and interview travel. See the full breakdown in USMLE exam fees and the total cost of the USMLE journey.
AMC exam fees (2026):
- AMC MCQ: AUD 2,920
- AMC Clinical Exam (in person): AUD 3,000
You will also pay for qualification checks, an English test, Ahpra registration, and travel to Melbourne for the Clinical Exam.
In exam fees alone, the two routes cost roughly the same. The bigger difference is time. The USMLE route usually takes longer before you start earning, which adds to the real cost.

How Long Does Each Route Take to Start Working?
USMLE: Most Indian doctors spend 12 to 18 months or more preparing for Step 1 and Step 2 CK. After that, they apply through the residency Match, which runs once a year. Residency usually starts in July. Residency itself lasts at least three years, depending on the specialty.
AMC: After passing the MCQ and the Clinical Exam, you receive the AMC Certificate. You can then apply to Ahpra for registration and start working in a supervised hospital job. You must complete 12 months of supervised practice, at least 47 weeks full time, before you can get general registration.
For many Indian doctors, the AMC route leads to a paid hospital job sooner.

Which Exam Should Indian Doctors Choose?
The USMLE may suit you if:
- You want to specialise through a structured residency program
- You are ready for a longer and more competitive process
- You are strong in basic sciences and can aim for a high Step 2 CK score
- You want to work in the US in the long term
The AMC may suit you if:
- You want to start working in a hospital sooner
- You prefer fewer exams
- You are comfortable with practical, patient based assessments
- You want to live and work in Australia
Some doctors also compare the USMLE with the UK route. The guide on USMLE vs PLAB covers that choice, and USMLE valid in which countries explains where a USMLE pass is recognised.
Conclusion
The USMLE and the AMC both open doors for Indian doctors, but they lead to very different careers. The AMC route usually has fewer exams and a faster start to paid work in Australia. The USMLE route is longer and more competitive, but it offers structured specialist training in the US. Before choosing, think about where you want to live, how long you can spend preparing, and your budget. A clear plan at the start saves time and money later.
