USMLE 2024: What is ECFMG Certification?

  • ECFMG assesses the readiness of international medical graduates (IMGs) to enter ACGME-accredited residency or fellowship programs in the United States.

New Look for ECFMG Certificate

ECFMG, a division of Intealth, has introduced a refreshed design for the ECFMG Certificate awarded to international medical graduates (IMGs) who complete ECFMG’s certification program. This update aligns the certificate’s appearance with Intealth’s brand identity, marking a significant step in our ongoing business transformation. 

Issuance of the redesigned certificates began in late February, maintaining the certificate’s significance and information integrity. The requirements for ECFMG Certification remain unchanged, assuring the U.S. public and graduate medical education (GME) community of IMGs’ readiness for supervised patient care in U.S. GME settings. 

Previously issued certificates remain valid, and no redesigns are issued to existing certificate holders. Organizations can verify an IMG’s ECFMG certification status through the Certification Verification Service (CVS), providing primary-source confirmation to various entities at no cost to U.S. residency/fellowship programs.

USMLE 2024: Requirement for ECFMG Certification

International medical graduates must meet requirements established by the Educational Commission for Foreign Medical Graduates (ECFMG) to be eligible for certification.

  • The physician’s medical school must be listed in the World Directory of Medical Schools (World Directory) with an ECFMG note stating that it meets eligibility requirements for students and graduates to apply to ECFMG for certification and examination.
  • Starting in 2024, ECFMG will begin implementing the ECFMG Recognized Accreditation Policy, linking certification to the accreditation status of a medical school, i.e ECFMG should be a sponsor of your medical school (or any other sponsor).
  • To be eligible for ECFMG certification, applicants must submit an Application for ECFMG Certification and pass Step 1 and Step 2 Clinical Knowledge of the United States Medical Licensing Examination (USMLE) or complete an ECFMG Pathway, including attaining a satisfactory score on the Occupational English Test (OET) Medicine, or have a passing performance on the former Step 2 Clinical Skills component of USMLE 2024.
  • There are time limits and other rules for completing the examination requirements for ECFMG certification.
  • International medical graduates must have been awarded credit for at least four credit years by a medical school that is listed in the World Directory with an ECFMG note stating it meets ECFMG eligibility requirements.
  • Applicants must document the completion of all requirements for, and receipt of, the final medical diploma and submit it to ECFMG for verification.
  • Submitting falsified or altered documents may result in a finding of irregular behaviour and permanent annotation of the applicant’s record.

USMLE 2024: Requesting an ECFMG Sponsor Note

  • An official of the medical school must submit a request to ECFMG for an ECFMG Sponsor Note
  • The official must send an email to medschoolreview@ecfmg.org to begin the process
  • When a request is received, ECFMG will evaluate whether the medical school meets the requirements to be granted an ECFMG Sponsor Note.
  • A request form will be provided and must be completed in full and signed by the Dean of the medical school, and the official seal of the medical school must be affixed to the form
  • The completed form and all required documentation must be returned to ECFMG
  • ECFMG will contact the appropriate government authority or authorities of the medical school’s country to confirm the school’s eligibility
  • The decision whether to grant an ECFMG Sponsor Note is made at the sole discretion of ECFMG and submission of a request does not guarantee it will be granted
  • ECFMG may determine at any time that a medical school no longer meets requirements and remove or modify the ECFMG Sponsor Note accordingly.

USMLE Step 1 Dates

  • Begin the registration process after enrolling in a medical college listed in the World Directory of Medical Schools.
  • Many candidates choose to schedule Step 1 exams after completing their second year of study.
